What Hospice at Home involves

Our Hospice at Home team provides expert nursing care in a family’s own home, for babies, children and young people whose condition is deteriorating or who are approaching the end of their life.

We work closely with families and community nursing teams, building a relationship and agreeing a shared plan of care. From managing symptoms to supporting a child at the end of life, our nurses and Advanced Clinical Practitioners provide the same high standard of care a family would receive at our hospice, in the comfort of their home.

For some families, this also means being able to bring their child home from hospital, with the confidence that expert help is close at hand.

When we can help

We’re here for families whose child has increasing care needs, whose condition is deteriorating or who want their child cared for at home in their final days. And because our team works seven days a week, that support is there whenever a family needs it.

 

Hospice at Home allowed for my son to be comfortable and cared for in his own home, that was one of his final wishes. I knew it was exactly what he wanted and was happy.

How to access Hospice at Home

A referral to Hospice at Home can be made by a healthcare professional, such as a hospital team, a community nurse or a GP. If you think the service could help your child, it’s worth talking to one of the professionals already involved in their care, who can start the process for you.

Once a referral is made, our team will get to know your family, understand your child’s needs and agree on a plan of care that works for you. From there, our nurses are only a phone call away. If there’s ever an urgent need, you can call us and ask for the Lead Nurse for Hospice at Home.
