Outreach at Rainbows

Rainbows recognises that the hospice is just too far away for some families to travel to and that there are some families who wish to receive hospice care and support but not in the hospice building. We are therefore working hard to explore how we can extend the care and support we provide to families closer to home.  We are introducing various services to respond to this need.  They include:

  • Neonatal service – Rainbows Neonatal Link Nurse is working closely with hospitals throughout the region to introduce Rainbows services and to support families
  • Inreach –  Rainbows family support team are increasing the support offered to families whilst in hospital in conjunction with the hospital teams
  • Local Activity Days – Days within local communities in conjunction with a local team - open to families known to us as well as those known by the local team.  Specialist activities and support
  • End of Life Sitting - Care within a family’s home when a child is end of life, carried out by a Hospice Care Assistant.  Service to enable the family to be at home but to ensure they can get adequate rest and  spend time all together whilst being supported in the care of their child
